One decision record
Record the decision, authority, rationale and any unresolved disagreement.
A small board needs a reliable way to turn discussion into decisions, actions and visible follow-up.
FICTIONAL EXAMPLE · Cedar Community Association
Select any record to see what makes it traceable. This is a fictional working pattern, not a client record.
Decisions were repeated, actions had no clear owner, and the secretary had to reconstruct the previous meeting from several email threads.
Use one decision log linked to an action register. Give each action an owner, a review point and evidence of completion. Keep disagreement in the decision record.
Record the decision, authority, rationale and any unresolved disagreement.
Use a named responsible role and a due or review date, rather than assigning work to “the committee”.
Link the completed output or record why an action was amended, paused or cancelled.
The fictional download contains a worked decision record, three linked actions and a short meeting-to-action operating guide.
This is a fictional worked example. Any dates, decisions and outputs in the sample are illustrative.
